Cost for the 2012 for each player is $1175.00.
Fees will be broken down to 3 payments.
This includes 7 tournaments, gym fee, coaches fee,and
club fee.
All 7 tournaments are 2 day tournaments.
The cost covers the coaches' hotel stays.
IT DOES NOT COVER THE PLAYERS'
OR PARENTS' STAYS.

We will have a hotel list and a block of 32 rooms for each
tournament located on the Hotel link soon.

Each team practices 2 times a week at St. Johns Country
Day School. Practices will start the first week of
December.

Fee also includes:
2 pairs Mizuno socks
2 pairs Muzino spandex
1 Mizuno warm-up pants and jacket
1 pair Mizuno kneepads
1 Mizuno bookbag
2 Mizuno jerseys
2 practice t-shirts
1 sweatshirt

1. 2012 medical release form- NOTORIZED
2. 2012 USAV Membership
(please choose Undecided as club choice)
3. Check for tryout fee-
$20 made out to North Florida Pirates
4.Check for $300 made out to North Florida Pirates
(check will be returned if you do not make the team)
